While more closely examining the rear carpet in my Fleetline, I found this between the jute insulation and the carpet itself near the torque tube tunnel. I seem to remember seeing one of these before somewhere, but thought I'd share this somewhat interesting find. It looks like maybe some problems were found with the right door and with the right side of the trunk lid. Looks like a 125 written at the top of the door...can't make out what is written by the wing vent, and a couple of 62s near the right side of the deck lid; something written at the bottom of the lid also that I can't make out.

That's very cool, Ralph!

I think I recall seeing the water testing done in one of the Jam Handy movies, but haven't seen one of these test tickets before.

I just took a look as some of the old videos. In the 1950 video "The Inside Story," there is a short segment showing the water test being done. Here's a link to one copy of the video here: The water test is at about the 8:15 point. As the worker is getting into the back seat, you can see that he has (what I assume to be) one of these forms, and a pen or pencil, in his left hand.
