YES, if you dropped something into the opening it could get to the valves & if small enough it could be on top of the piston now.
Depending on the type of metal, you might be able to use a magnet in the spark plug hole & find it.

If you find it and it is just a little beat up, it may not of done any real damage, test drive would let you know for sure. Otherwise you will need to pull the heads to find it, a washer would likely not cause any real damage, but a nut is much harder & could cause some issues.
