The tech help at Cardone sent it to me. Maybe if you called them they could do same for a 4 terminal motor. Their number is 888-280-TECH (8324), Mon - Thurs 8am-6pm, Fri 8am-5pm EST. If it will help you, I'll send the drawing I have to you if you give me email address. Tried to upload it in this reply but was unable to get it done.
