If you don't use the e-brake, the adjuster sticks, not the piston.

There's a couple of holes in the face of the piston. A special spanner with a pair of pins that engage those holes works to rotate it to get the adjustment "close". You can make one, or maybe you can just use channel locks if the piston sticks up high enough.
"I know about the ratcheting mechanism because it was too tight to start and the wheels wouldn't turn so I loosened 'em up just enough to get the wheels to turn."
Yeah, same thing, what I'm saying is that if it's too loose you turn it the other way.
"If I pull the E-brake just a bit the brakes seem to grab better but I don't think that's a good way to go."
If the e-brake is working, try applying it and releasing it completely a bunch of times and see if that helps. You're right, even if driving around with the e-brake cables partially pulled helped, it wouldn't be a good long term thing.
